Notification of the Department of Labor Protection and Welfare Re: Forms for Application, Application Receipt, License, and Substitute License for Occupational Safety, Health, and Environment Experts

Key Content: This notification prescribes the official forms related to Occupational Safety, Health, and Environment (OSHE) Experts as follows:

  1. Application Form: An application for a license, substitute license, or license renewal as an OSHE Expert shall be in accordance with Form กภ.คบญ.33.

  2. Application Receipt: The receipt of application for an OSHE Expert shall be in accordance with Form กภ.รบญ.33.

  3. License Form: The license for an OSHE Expert shall be in accordance with Forms กภ.บญ.33 – 1 and กภ.บญ.33 – 2.

  4. Substitute License Form: The substitute license for an OSHE Expert shall be in accordance with Forms กภ.บทญ.33 – 1 and กภ.บทญ.33 – 2.
